Health forms for massage, facials and wellness

For massage, facials, and any wellness service you can attach a health intake form. Customers sign it in advance from their account; therapists see a clean summary on the booking.

Add the health preset

• Go to App › Services and open the service.
• Scroll to Intake fields and click + Add health form preset.
• Review the questions (allergies, pregnancy, medication, injuries, conditions, confirmation checkbox), tweak as needed, then Save fields.

Customer experience

• After booking, the customer is asked to complete the intake form.
• They sign with their name; the signed record is saved and can be exported as a PDF.
• The therapist sees the answers on the booking detail before the appointment.

Rooms as resources

For spas that book by room (couples suite, salt cave, steam room), assign a resource to the service so the calendar prevents double-booking the room even when two therapists are on shift.
